* If you have a patient referral, you may:
* Send your referral through Infina Connect        OR
* Fax your referral form to 877-745-2672 OR 919-582-7274
* Please include the last office visit note, demographic information, and insurance information.

If you require us to fax back the visit once the patient has been seen please include that on your cover sheet. We look forward to working with you.
